Job Description Of Team Leader
- Complete all service user assessment, including risk assessments as part of the assessment process, for new service users.
- As required by the company’s policies, routinely monitor, review, and update the care plan for the client.
- To participate in a rotating system of being on call in case of emergency.
- Should communicate often with care coordinators regarding hiring, distributing care workers, and developing care & support.
- To keep a close line of communication open with the Care Coordinator and Registered Manager.
- Support all field-based carer workers and ensure care workers in the field are observed, supervised, and mentored.
- When a current care worker reports that they won’t be able to make to attend to a call, assign a suitable replacement care worker.
- To make sure that all written reports, administration, and data kept in a service user’ homes are accurate and current.
- Make sure all client visits are always covered.
- Conduct new care worker orientation visits with clients.
- Immediately notify the care coordinators of any alterations to a care worker or service user’s schedule.
